Dear parents and caregivers of Windsor School,

As we are approaching the end of the school year, we are sending this letter as a reminder to parents to pick up their child’s medication by the last day of school, June 19th. By school policy, we are required to discard/destroy all remaining medications--both prescription and over-the-counter which have not been taken home. We are unable to store medications for the following year.

A quick reminder for next year--all over-the-counter medications must be in the original bottle/package with the expiration date. The medication must be labeled with the child’s name. All prescription medication must be in the original pharmacy container with correct information on the label. All medications, must be accompanied by a “Request for Medication Administration during School Hours” form, with parent/guardian’s signature. As well as a signed doctor’s order for administration.. Medications will only be administered at school the specified times noted on the form. Any changes in dose, time or medication will require a new form, & a new container with the corrected pharmacy label.

If parents wish for their child to carry emergency medication on them, such as inhalers and epipens, there are steps that need to be taken.  Both the medication & the medication form must be checked in at the Nurse’s Office at the beginning of the year. The physician must check “yes” on the form stating “Student may carry medication for Emergency purposes”.  We really need these emergency medications on the first day of school. Medication administration forms must be renewed annually, and will be available at open house or by contacting the school nurse. The form and medication may be brought to school in the fall prior to the first day of class. Thank you for your cooperation. Have a wonderful summer!
